Respite Care

Respite care is a service that provides supportive care and supervision to recipients under the age of 21 when the primary caregiver is unable to perform the duties of a caregiver. This service is generally used due to a brief planned or emergency absence, or when the primary caregiver is available, but temporarily physically unable to care for or supervise the recipient for a brief period of time. Respite care is not intended to be used as after school care or supplement personal care assistance when daily limits for personal care assistance is exceeded.

  • PLACE OF SERVICE: Respite can be provided in the recipient’s family home, while involved with activities in the community, in a licensed group home, foster home or assisted living facility. 

CDC PLUS SERVICES & GR FUNDS

LONG TERM CARE (LTC) 

CDC + Services

CDC+ is a long-term care program alternative to the Medicaid Home and Community-Based Services (HCBS) Medicaid Waiver. The program provides the opportunity for individuals to improve the quality of their lives by being empowered to make choices about the supports and services that will meet their long-term care needs and to help them reach their goals.

GR FUNDS

GR funds is a temporary emergency fund that is provided by APD WAIVER. 

PERSONAL CARE SERVICES- PEDIATRICS

Personal Care Services 

To provide medically necessary assistance with activities of daily living that support a recipient’s medical care needs. Florida Medicaid reimburses for PCS for qualified recipients : Have complex medical problems and require more individual and continuous care than can be provided through a home health aide visit. To assist with activites of daily living. Currently only providing these services in the following counties: ORANGE, SEMINOLE, BREVARD & OSCEOLA

LTC HOMEMAKER & COMPANION SERVICES

 

Homemaker & Companion Services Providers employ individuals to do housekeeping, cook, run errands, and provide companionship to the elderly and adults with disabilities. They cannot provide any personal care or nursing according to state law. Currently only providing these services in the following counties: ORANGE, SEMINOLE, BREVARD & OSCEOLA
